Table view of last call comments

In order to get a better overview of the areas of ATAG 2.0 that require 
attention, I have prepared a table view of the comments (attached).

Note that the non-glossary EOWG and IBM comments are grouped into just two 
bugs for now (1197 and 1196), but I have split them out into the relevant 
document sections in the table view.

I hope people find this helpful.
