In 4.3.2 of Part 1, we have the following:

	Relative URLs in entity declarations within the internal DTD 
	subset are thus relative to the location of the document

What happens if "the document entity might well appear on an input stream
of the processor without any identification at all"?  (Part 1, 4.3.4)
