Re: IDs - make them case sensitive

Despite the fact that Unicode grows, XML ID names must reside in a
finite list of "name characters". This is not the case in HTML. XML also
specifies the relationship between lower case and upper case versions of
name characters. Both of these are in the XML declaration.
