Last I knew we were supposed to follow the Chicago Manual of Style,
which seems to want American-style capitalization - Table of Contents,
Status of this Document. I always think it looks quaint and silly, e.g.
in the newspapers, but that's beside the point :)

My printed copy of the Chicago Manual of Style is in storage right now,
though.
