DTD update in XML Signature 1.1, Encryption 1.1 and Signature 2.0

I removed the optional URL  from the DTD DOCTYPE of all the preamble examples in XML Signature 1.1, XML Encryption 1.1 and XML Signature 2.0.

I suggest we retain the DTD information otherwise.
