ACTION-286: Remove references to DTDs

I've deleted all of the DTD sections in the xmlsig-11 draft and edited some
section titles. I added this to a "general changes" section in the explain
document rather than have to repeat the change across every section.

I reviewed the xmlenc-11 draft and there don't appear to be DTDs in that
document.
