Low hanging fruit?

Do any of these changes need additional discussion?

1. Add p:empty as a peer to p:inline/p:document/p:pipe to explicitly
   set the context to an empty document sequence

2. Make the default readable port the default input. (I think this is
   just an oversight as Jeni suggested.)

3. We're going to use functions, not variables, to access pipeline state.
   (i.e., p:episode() not $p:episode).

4. We can replace p:episode() with p:system-property as I proposed a
   few days ago?

5. Drop the p:tee step
