Default bindings on p:output incoherent???

I can't see how, if at all, the current spec. disallows e.g.

 <p:declare-step name="my:step">
  ...
  <p:output>
   <p:pipe step="a" port="b"/>
  </p:output>


I think we need to say that when p:output appears inside
p:declare-step, it must be empty/contain only documentation.
