Unconnected primary output port

I can't find where the spec says that the primary output port of a
step must be (explicitly or implicitly) connected.  I see mail from
Jeni pointing out that it used to say that all of the outputs must be
connected (which was wrong), but now we seem to have lost the
constraint on primary outputs.
