My changes:

* defined a minimum set of serialization options for the 'xml' method per the
   serialization spec
* made serialization options their own section
* added a note about use-character-maps
* made wrap-sequence have sequence output
* added more detail about xsl-formatter output media type and
   parameters input
* added an error for xsl:message/@terminate='yes' in XSLT 1 & 2
* clarified the base URI of step outputs
* add p:make-absolute-uris and p:add-xml-base
