I understand that this is a fundamental principle in XProc, makes life
much easier for implementation as well....but the scenario I am
describing is common.

perhaps all that is required is amending <p:wrap> to optionally apply

     <p:option name="wrapper" required="yes|no|auto"/>

where the @required = 'auto'
