Just checking that we agree on this.

According to the text, this is how you evaluate the value of an <item>, in  
order of precedence:

1. item@ref or item@bind
2. item/value@ref item/value@bind
3. item/value@value
4. item/value:content
5. item@value

(and my last email suggested:

6. item:content
)
