Documentation of Issue 5 and 6

Jacek,

Issue 5 is now documented in section 2.  Examples have not been updated,
but that will be done when we further restructure section 2 once the rest
of schema mapping and related issues become more clear.

The part of issue 6 we resolved is also documented in section 2.
