Issues 26 and 27 provisionally documented.

The spec now contains provisional resolutions to 26 and 27.  I will adjust
as necessary before the document freeze on Wednesday evening.  The document
also includes other editorial improvements including changes to address the
editorial comments I recieved from Jaceck.
