Hi all,

In preparation for the publication of the wsdl 11 element identifiers
draft I made some changes:

- Put the status section in a separate file status-wsdl11ei.xml
- Changed the reference from wsdl11eientities.dtd to entities.dtd and
updated various entities in entities.dtd (makes usage of existing make
tools easier
- Updated various entity references in the text of
wsdl11elementidentifiers.xml
- Changed the references from <bibref/> to [<bibref/>]
- Changed the usage of <code> elements to allow for emphasis within them
via <code role="code-emph"> ... , and changed xmlspec-policy.xsl accordingly
- Reformatted the example in sec. 3.4
- Updated the bibliography
- Added Makefile.wsdl11ei and updated the main makefile (not important
for the editors, only for TR space publication)
