Then, your changes didn't make into CVS [1]. These changes aren't in revisions 1.79 to 1.81.

I have already done that and checked in the attachment document with this change. I do not understand why you needed to do this again on a completed action item. 
 
Is the CVS not working and you are not getting the document that is checked into the repository?

In my previous check in, I added 'wsp' prefix to example 5.2, Attachment draft, and all occurrences of <el>Policy</el> and <el>PolicyReference</el>.
