delimiter or not in default fault action example

In section 4.2.4, the default fault action pattern is given as

 ...[operation name][delimiter]Fault[delimiter][fault name]

but when listing the applied patterns, the fault action does not have the 
delimiter in front of the "Fault", probably just missing here!?
