I was pleasantly surprised that the issue has been already raised...
I think that the proposed resolution #1 has an added benefit: since header 
entries may appear as MAPs in arbitrary order, if wsa:To is an EPR, it 
will naturally group any [reference parameters] and thus make it possible 
for some processors to achieve extra efficiency. They will not have to 
inspect the entire Header in order to make sure that there are no 
reference parameters to take into account.
