Does this mean that the entity using an EPR has to "rewrite" it when 
serializing it into a SOAP header, to add the appropriate attribute?

That is going to make it very difficult for an EPR minter to sign an 
EPR.  At a minimum, it would require the "signing EPR minter" to be 
aware of what SOAP version the using-entity is, well, using.
