Regarding the manifest spec you're referring to: this document is an old deprecated draft, it is not maintained anymore.
The latest manifest draft is http://www.w3.org/2008/webapps/manifest/, it is still being worked on by the WebApps WG.
The SysApp WG plans to formalize the trust model of System Applications (= web applications with special privileges), but nothing yet is specified, and one cannot assume it will use permissions.
