I wrote a little bit about how an implementation might implement respondWith() using a js ReadableStream in https://github.com/w3c/ServiceWorker/issues/1168#issuecomment-317031289 and https://github.com/w3c/ServiceWorker/issues/1168#issuecomment-317032758.
