Re: [whatwg/xhr] setRequestHeader behavior doesn't seem to match implementations (#108)

@youennf so in https://github.com/whatwg/fetch/issues/189 you asked for header ordering to be defined as lexicographical. However, what do you suggest we do for `getAllResponseHeaders()`? What WebKit implements at the moment doesn't seem like an option.
