[w3c/webcomponents] Create an element type extension path with sync flag set should not enqueue upgrade; it should just upgrade (#469)

This seems complicated... Maybe we should split "create an element" into two algorithms, one with type extensions and one without. I think all call sites would be able to adapt pretty simply.
