[w3c/webcomponents] create an element does not always return result (#467)

Step 3.4.3 and 3.4.5 should become step 3.6 and 3.7. Step 3.5.1 needs to be merged into step 3.5 and step 3.5.2 can be removed (leaving 3.5 with no substeps).
