[w3c/webcomponents] Mutation: insert (#465)

TL;DR: change the conditional to "If _inclusiveDescendant_ is in a shadow-including document".

1. "was previously not" is not a primitive.
2. It could have been previously in such a document, however, when it was disconnected there it would have its `disconnectedCallback` fired. So we should fire its `connectedCallback` here.
