The <body> cases appear to be covered by the current spec text, though I still
need to run through the algorithm one more time looking for gaps. Barring any
discoveries there, this should just require one additional sub-section:
Addition to the "in head" Insertion Mode.

Any other specific additions you were thinking of here?
