We could also harmonize the structure of the reference section entries.
In my documents I use the following:

<dt>[label]</dt>
<dd><a href="referenced_document">title</a> - authors. type of document 
and date.</dd>

e.g.:

Also: Are the brackets in the in-text references to a referenced 
document (e.g. [XML]) part of the link?
