Re: BAD "after" template ready

Please find several suggestions for the "after" survey form:

* Document title should read "Citylights Survey" rather than "Citylights template".
* Copy and paste the right-hand side bar from the index page (not the empty template).
* Remove link target from left menu bar to avoid self-referencial links (as on index).
* Remove hover effect on the fieldsets since this is not available on the before page.
* Keep red color for required fields (as on before page) but add redundant information.
* Ensure that the content (headings and questions) are the same as on the before page.
