new draft of form_before page

Hi Shadi, hi all.
Here's new draft of form_before page.

About features list:
- nested layout tables that do not linearize correctly.
done; I used a transparent gif too.

- incorrect label elements for form controls.
	- no label elements for some controls
	- label elements have no "for" attribute
done.

- color coding for required input (i.e. "all fields in red should be  
filled in")
done.

- acronyms and abbreviations are not marked up at their first occurence.
There are no acronyms and abbreviations; so I thought to use <th> to  
simulate bold text for questions in table (Where do you live, etc.).
