[SKOS] "Documentary Notes" and other section headings

Or: "Documentary Notes" (also in SKOS Reference).  "Notes"
fits better with Concepts, Labels, Relationships, and Schemes.

A few other section headings could perhaps be improved.
In Section 1.1:

    a. "Document structure and reading guide"      -> drop...
    b. "Examples" -> change to "About examples in this primer"

With generic names, the reader might expect to documentation
about SKOS in 2.4, something about structured documents in
1.1a, and some actual examples in 1.1b...
