[SKOS] issues management

To bring the content on the wiki and the SKOS web site into line with
the decision to use the Issues Tracker to manage SKOS issues, I've made
changes to the following resources:

http://www.w3.org/2006/07/SWD/wiki/SkosIssues
http://www.w3.org/2006/07/SWD/wiki/SkosIssuesSandbox

Ralph has proposed to update the content of the following resource to be
consistent with our current issues process:

http://www.w3.org/2006/07/SWD/wiki/IssuesProcess

... I suggest he go ahead with this.
