revised transition request attached

Some changes based on an overnight review with Henry.  I think all are 
cosmetic for those in the wg, but by all means holler if not.
- Re-worded to clarify: changes since last transition, formal objections
- Re-formatted list of URIs at the bottom
- Added strikeout text (that we authorized the editors to add back in if 
necessary for pubrules during the telecon yesterday) to the word-comment 
(which renders in html as a footnote)
