Initial Writeup for ACPP Coronary Artery Bypass Guidelines Usecase

An initial writeup is available at the link below which gives a brief 
background of the purpose of the report that the usecase is based on 
(ACC/AHA Guidelines and Indications for Coronary Artery Bypass Graft Surgery), it's 
relevance to decision support, the risk factors it depends on (the driving 
patient data) and other some other possibilities (such as the possibility 
of accomodating uncertainty)

It is still a work in progress, but comments are welcome.
