[RIF] generated frozen versions of documents

The "Set of Documents" section looks wrong for some of the documents:
UCR, BLD, DTB, and PRD are each in a set of 2, and SWC is missing that 
section.

The show/hide code example buttons in UCR section 4 don't work in the 
frozen version.

 BLD (not in wiki version) has some unconventionally numbered lists (eg 1 
3  4  6  8  9 10 ...)
 in sections 3.2, 3.4 and 6.1 (both the main list and sub list for 6.1).
