Lookup table proposal (re ISSUE-72)

In the interest of getting R2RML 1.0 out, and since we clearly do not have
consensus on a solution (and maybe not even on the problem) I propose the
following:

PROPOSAL: Resolve ISSUE-72 by removing the lookup table facility from the
R2RML spec.
