[Bug 1686] Hard to find the definitions of purely symbolic judgments

Here I'll add what may be the best solution of all:
add an appendix with a master list of all judgments, preferably in
alphabetic order (with the symbolic ones such as <: or =>stat
sorted in some intelligible fashion), and with hot links to 
the definition.  Place a reference to this list in 2.1.2 
"Notations for judgments" for good measure.
