The XQuery book no longer speaks of DTDs at all - it's in the Data Model
document, which says, "All general and external parsed entities must be
fully expanded" (in section 3.2).

The XQuery WG feels the XQuery specifiation no longer neesd such a
pointer since the text in which it wouldhave appeared has been removed.
