DN-FO-05: 15.1.6 fn:item-at -- examples

The first example is incomplete because $item1 is not defined (please, note
that a reader may visit this section of the document directly through the
corresponding link in the TOC, being therefore unaware of the source xml
documents and variable bindings provided in a parent section).

Suggested solution:
Make all examples self-contained, or add at the start of all examples a link
to the missing definitions.
