Andrew,
anySimpleType is not the same as string. For example, arithmetic can be 
performed on values of anySimpleType, but it cannot be performed on 
strings.

In numbered point 1. in 2.3.2 I wondered why the type is xsd:anySimpleType 

rather than xsd:String.

If it is a text node, then isn't it xsd:String?
