Filled in Surveys

Hi Stephan, Paul.
As I agreed in the last teleconference, I filled 2 surveys: 1 implemenation
and 1 extension vocabulary.

I know it may be late for comments/suggestions now, but here goes my
impressions:

   1. Is the rationale box supposed to explain the rationale for every
   concept? I have just added some remarks there.
   2. In the extension survey you have the options: 1)Don't know,
   2)Directly used, 3) Extended. What if I don't extend
   a particular term? (This option was present on the implementors survey).
   I left the "No opinion" on the terms
   that I had not extended.
