Can you add one or more items to
https://www.w3.org/community/ppl/wiki/CustomerRequirements so our record
of these use cases isn't lost in the email archives?

The whole extension function idea is a polyfill since XSL-FO 2.0 didn't
get to the point of having copyfitting implementations in FO formatters
beyond an experimental FOP extension at University of Bologna, so why not
use a polyfill to make another polyfill?
