Ah, that one is easy. It can be either one of two things:
* Someone assisted the scribe by quoting what someone said on the  
phone. This is done by typing a name, followed by a colon, and then  
the text (e.g. Michael: What are these "Scribe assist by...]" entries?)
* Or someone accidentally used a colon in a sentence
