ACTION-141 done: clarify the role of the UA for rendering a media fragment

Hi,

I've rewritten the intro to the section "Resolving URI fragments
within the user agent" to cover the general case where the user agent
can extract a fragment locally. I've also added an example using
adaptive streaming representations, where the UA is not necessarily
doing byte range requests on the original resource.
