Updates the spec per the [defining a new media type][1] extensibility 
guidelines. See the [detailed breakdown of changes][2] and the [RawGit
 preview of the spec with these changes][3].

@huningxin Please review.

@burnburn These guidelines ([related issue][4]) were very helpful, 
thanks for the initial version. My [comments][2] may help refine them.

Fixes #92. (Defining a new constrainable property guidelines were 
implemented in a separete PR #97.)
