New editor's copy of Unicode in XML and other Markup Languages

I have just finished adding Rick McGowan's suggested edits to the 
document, and a few other tweaks to bring the document up to date as a 
base for exploring further editorial and substantive changes prior to 
publication.

Please take a look an suggest any additional changes that may be necessary:

a. to complete the refresh

b. to add/change substantive information
