Review table format

I've been messing with the review comment table format again.

You can now add a subject lead to a span near the top of the page that will be included in the search string. This will reduce the number of returned emails - which was a problem for the WAI stuff I was just working on.

I also added another column to the right, again requiring minimum maintenance. If you add a class of 'closed' to a cell in this column it will turn a nice colour to indicate that the comment has been resolved, without you having to follow the links to check.

See for example http://www.w3.org/International/reviews/0708-smil30/ which is also useful as it shows at a glance that we have 4 unresolved issues to track down with the SMIL folks.

I've added these features to just a few of the most recently used tables.

Enjoy,

RI
