[MSE] Spec updates & FPWD candidate published

Hi,

I have checked in changes to the Media Source
Extensions<http://dvcs.w3.org/hg/html-media/raw-file/tip/media-source/media-source.html>
that
Adrian and I believe resolve all the outstanding issues for publishing a
FPWD.

I have generated a Media Source Extensions FPWD
candidate<http://dvcs.w3.org/hg/html-media/raw-file/tip/media-source/media-source-fpwd.html>
for
review by the group. I've set the publish date to January 24th under the
assumption that the media group CfC on Tuesday succeeds and a 2 week CfC is
started in the HTML-WG on the Thursday the 10th.

Summary of the changes:
- Renamed append() to appendArrayBuffer() and made appending asynchronous. (Bug

- Added SourceBuffer.appendStream(). (Bug
18962<https://www.w3.org/Bugs/Public/show_bug.cgi?id=18962>
)
- Added SourceBuffer.setTrackInfo() methods. (Bug
17006<https://www.w3.org/Bugs/Public/show_bug.cgi?id=17006>
)
- Added issue boxes to relevant sections for outstanding bugs.
