Resolved along the lines of comment #9 here:

I did *not* add a requirement about any change that affects an approved test
case. "particularly exceptional circumstances" applied to feature additions, so
I addressed that. If anyone believes we should tighten the requirements on any
change that affects a test case, then I suggest filing a follow-up bug.
