[Bug 12897] UTF-8 BOM should trump users and/or HTTP (Encoding sniffing algorithm)

UTF-8 encoded HTML-compatible XHTML file with BOM, served as 'Content-Type:
text/html; charset=koi8-r'

Parsers which follow the recommendation in  XML 1.0, should respect the BOM and
ignore the encoding information inisde the Content-Type header.
