recursion/layering tests

I worked on tests that stack XML/HTML/HTML etc using namespace docs and 
profile docs.

The results can be found at:

(I selected three of the 512 such tests I think I added!)


The todo list

still has quite a lot in it.
We might want to pare it down somewhat.

I'll next try some of the following:

+ multiple namespace transformations

+ Passing an XSLT 2.0 transformation to an XSLT 1.0 engine

+ includes "", the same-document URI reference.

+ test use of grddl:transformation attribute in conjuction with 
namespace document(s), XHTML profile(s)
